Thank you for submitting your intake form.

Next Steps

  1. When we receive the other party's intake form we will get in contact with you to book the phone intake assessment.

  2. On that call, we’ll ask a few more questions about your child, your situation, and the days/times you’d like services to occur.

  3. After the call, you’ll receive a Service Agreement for signature outlining roles and responsibilities for all parties, payment terms, our cancellation policy, and reasons services may be terminated.

  4. Then we’ll be able to allocate a supervisor and arrange the visits or changeovers​​
